Why does one side lead the other?
Because the two drive points have lost synchronisation, and on most systems that is adjustable. A slide entering the opening at an angle is both the symptom and the cause of further damage, because it loads one side of the mechanism far harder than the other.
Resynchronising is a straightforward job. Leaving it is what turns an adjustment into a mechanism replacement.
